In case you are on annual leave or sickness etc, please provide a secondary contact for us to use, should we not be able to get hold of you. Please be aware that repeated failed attempts may result in your donations going to other causes.

Please bear in mind that filling in this form, does not guarantee you Toy Donations. Helena and her Team will do their best to allocate on a first come, first served basis.
